Participation Requirements

• Student must be passing 5 classes
• Current Physical-within the last 2 years, unless there is a change in medical history-Required upon entering 7th and 9th grade
  • Pre-participation Physical Form
  • Interim Medical Form (in ATS below) needs to be filled out on the physical off year.
• Participation Fees
  • $40.00 for first extracurricular activity
  • $70.00 for two or more activities
  • Students who qualify for free or reduced school lunch shall be granted a 50% reduction of the participation fees.

• Student Athlete & Parent/Legal Guardian Concussion Statement
• Concussion Form
• Helmet Form
• Transportation Request & Release Form  (required permission form for students to ride with a parent/guardian when returning from any school-sponsored event - must be signed & approved one week prior to leaving for event)
